Why Participate?

Example Prompt & Dashboard

You are a pharmacist working in a multidisciplinary outpatient clinic and participate on a transitions of care (TOC) team. One of the departmental goals is to reduce readmission rates.

Your clinic director emails you saying, "It feels like our readmission rates have been increasing, but I don't have timely data to determine if this is the case and where our gaps might be. I'd like each group to rotate presenting during our monthly all-team meeting about their initiatives and metrics.

Can you represent the pharmacy team and do the following?

  • Create a 1-page dashboard that includes the current 30-day readmission rate and the monthly readmission trend.

  • You're the pharmacy expert, so can you create a custom key performance indicator (KPIs) or chart to highlight a pharmacy relevant metric? Please include your metric target to make it easier to evaluate performance.

  • Is our limited staffing hurting our wait times to get into clinic? Our goal is for patients to be seen within 2 days of discharge. I'd love for pharmacy to play a more active role during the visits to make them more efficient. Can you incorporate this into your dashboard as well?

Our IT team pulled some data about relevant patient visits, their most recent discharge, and key demographic information. The files on the secure drive will also include definitions to orient you to the files. Thank you again for your help!"
